Request a replacement certificate or card

    To request a replacement certificate and/or card, please complete and submit the form below.  A member of our team will then email you an invoice with the payment details (we only accept cheque or bank transfer).

    Due to data protection legislation, we will also need to see evidence of your ID e.g., a copy of your photo driving licence, passport, or a recent utility bill (this evidence will be immediately destroyed after verification).

    On receipt of payment and evidence of your ID, your replacement certificate or card will be posted out to you.
     
    Certificate replacement - £50 plus VAT (£60)
    Card replacement - £25 plus VAT (£30)
