Request a replacement certificate, card or operative report*

    To request a replacement certificate, card or operative report*, please complete and submit the form below.  A member of our team will then email you an invoice with the payment details (we only accept cheque or bank transfer).

    *Operative reports are only available to candidates that are returning to take their assessments at a Cert-ain Certification Ltd. approved assessment centre.

    Due to data protection legislation, we will also need to see evidence of your ID e.g., a copy of your photo driving licence, passport, or a recent utility bill (this evidence will be immediately destroyed after verification).

    On receipt of payment and evidence of your ID, your replacement certificate, card or operative report will be issued to you.
     
    Certificate replacement - £55 plus VAT (£66)
    Card replacement - £25 plus VAT (£30)
    Operative report - £15 plus VAT (£18)
